New Patient Registration
Our policy is that we have to obtain two forms of identification from anyone who wishes to join our practice. These are:
- Photographic evidence of identification.
- Recent utility bill or some other evidence of current address.
A named accountable GP is allocated to all patients (including children), who is responsible for patients' overall care at the practice.
It is necessary for all new patients to book an appointment for a health screening examination and to complete a health questionnaire.
We regret that we are unable to accept new patients outside the area indicated on our map.
If you wish to register click on the links below to print off a Registration Form and a New Patient Questionnaire. When you have completed both of these forms bring them with you when visit the surgery for the first time, along with two forms of ID (one photographic and one confirming your address).
Patients who have arrived from abroad and are wishing to register the following ID will be necessary:
- Visa or permit to stay in the country
- In some circumstances other forms of ID will be accepted.
