Social Prescriber: Louise Brearley
Louise works closely with the GPs identifying and supporting our most vulnerable patients. Her role enables her to work for a wide range of people, including; patient with one or more long term conditions, patients who who need support with their mental health issues, patient who are lonely and isolated, and patients who have complex social health needs affecting their wellbeing.

Practice Pharmacist: Sarah Siviter
Sarah’s main role as our Pharmacist is to ensure our patients get optimum benefit from their medication. If you have any questions about your medication, or would like to review your medication with Sarah, please contact our Reception team.
The Care Navigator Team works closely with the GPs, District Nurses and Adult & Community Services. They offer non-clinical support for our most vulnerable patients and their social needs.

The Health Visiting Team are at the surgery on the second Tuesday of every month from 13:30 - 15:30 (subject to change) to run the Baby Clinics. These clinics are held on the ground floor of Ringwood Medical Centre.
