Register as New Patient

Our policy is that we have to obtain two forms of identification from anyone who wishes to join our practice. These are:

  1. Photographic evidence of identification.
  2. Recent utility bill or some other evidence of current address.

It is necessary for all new patients to complete a health questionnaire. A named accountable GP is allocated to all patients, including children, who are responsible for patients' overall care at the practice.

If you wish to register click on the links below to print off a Registration Form and a New Patient Questionnaire. When you have completed both of these forms bring them with you when visit the surgery for the first time, along with your two forms of ID.

We regret that we are unable to accept new patients outside the area indicated on our map.

Changes to Your Personal Details

It is your responsibility to keep us informed of any changes to your contact information. Please advise us if there are any changes to your name, address or telephone number. 

It would helpful to ensure that we have your mobile telephone number as we offer an appointment confirmation and reminder SMS service. Keeping your information up-to-date ensures we are always contacting the right person.

If you are moving home to a new location outside our practice boundary, you will be required to register with a GP in that area as soon as possible.


Temporary Patient Registrations

If you are unwell while away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days.

After 14 days you will need to register as a temporary patient, this will last up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary  patient at a practice in the town or area where you are already registered.

We aim to make the surgery as accessible as we can for all our patients.

This includes:

  • Disabled Parking and Access*
  • Guide Dog Friendly
  • Large Print Letters

We are happy to help with any assistance you require.

*There are three disabled parking bays for patient accessibility and a ‘drop off’ area outside the main entrance for those patients finding access difficult. 

Please note that due to the limited amount of space the surgery car park is for GPs and Staff only. Ringwood offers three Pay & Display car parks, all within a short walking distance of the Surgery.
